- Serve as the primary point of contact for patients, families, and healthcare providers, addressing inquiries and concerns with empathy and professionalism.
- Develop personalized care plans with medical staff and other professionals to meet each patient’s unique needs.
- Implement health promotion initiatives and wellness programs to educate patients about preventive care.
- Monitor patient progress and outcomes, adjusting care plans as necessary to achieve optimal results.
- Maintain accurate patient records and documentation by legal requirements and organizational policies.
- Work closely with multidisciplinary teams to ensure seamless service delivery across all levels of care.
- Participate in community outreach programs to promote CIC Lifecare’s services and enhance community health awareness.
Qualifications:
- Our staff is well qualified, like BSc Nursing, GNM, ANM, and OT Technician.
- In-depth knowledge of healthcare policies, regulations, and industry best practices.
- Exceptional communication skills with a knack for building rapport with patients and colleagues.
- Adept at problem-solving with strong organizational skills to manage multiple priorities effectively.
